What Needs to be Completed and Uploaded

Step 1

Download and complete the IEEE SA Standards Fellowship Program Registration Form (PDF). Once completed, please upload it, along with all the additional documentation required in steps 2-3.

Be sure to fill out the fields in the form below and acknowledge the IEEE SA privacy policy, when you are ready to upload your documents.

Step 2

Required documentation that you will need to provide and upload:

  1. Your Brief Biography
    This should be no more than 300 words summarizing your professional experience, including a short paragraph highlighting your expectations for this program and what you are looking to achieve by participating in this program.

    Please refrain from uploading CVs as they will not be accepted.
  2. Your Photo
    Submit a high-resolution professional photograph of yourself.

    Accepted file types: jpg, jpeg, gif, png, Max. file size: 4 MB.
  3. Travel Preferences
    Please provide two flight itineraries arriving at/departing from the airport nearest to the meeting location in order of preference.

Step 3

Required documentation that you will need to provide and upload:

  1. Vendor Add Form and Substitute Form W-8BEN (PDF)
    Download and complete sections 1-4 (Part I) and section 9 (Part II) before uploading the signed and dated (Part III) document.

    This is a legal tax document required by the US Internal Revenue Service to declare your US tax status.

  2. A valid visa
    Please provide a copy of your visa. Accepted file types: pdf, doc, docx, jpg, gif

    IMPORTANT: IEEE SA will not make any travel arrangements for you until proof of visa has been provided. If you do not have a valid visa yet, please bookmark this page (Ctrl + D – Windows / Cmd + D – Mac) so that you can upload it when you receive it.

  3. Wire Transfer/ACH Payment Form (PDF)
    Please download to complete.

    IEEE SA will directly purchase reasonable economy airfare, airport transit, and lodging for the participant to participate in this IEEE SA Standards Fellowship Program.

    Food cost and daily ground transportation to/from IEEE offices will be covered by a per diem which will be sent to the participant prior to the meeting.

    IMPORTANT:  For this purpose, the IEEE Wire Transfer/ACH Payment Form needs to be completed from “Bank Name” to “Bank Account or IBAN#” (see screenshot). Please note, the per diem will be issued upon proof of the relevant approved US visa (see bullet above).

    Note: Once you have completed and uploaded all of the documentation above, you will be sent your IEEE SA Standards Fellowship Program Participant Agreement electronically, for your review and signature. A sample of this document is listed below:
